I'm not positive when an update was pushed out that caused this - my
machine gets updates automatically every night but I rarely reboot. I
rebooted yesterday and was immediately affected by it.

Actually one more comment - I was digging into my auth.log and it may be

are identical. I then noticed that one of my three monitors shows

Thanks for that tip Michael. Sad, but after updating to 16.04 the
problem persists. I can't seem to find a permanent fix.
I've also changed video cards (nvidia 960 now) so the vid card itself
apparently wasn't involved. This is an upgrade install all the way from

Ah I found another workaround / hack someone left on a comment in my
original askubuntu question.
It's definitely a hack but it got me around this bug. I extracted the

Robert where are you specifying the filesystem.squashfs file? In the

Are you providing any other options in the ks file to enable the proxy

I still get the same error, (prefixed with /cdrom) even with inserting

Has anyone been able to come up with a workaround for this yet? I'm
completely stumped. I even tried the net installer from 14.04 to no
avail.

Attempting to use PXE to install 16.04 results in an error dialog that
states "The failing step is: Install the system". Checking syslog shows
that it has prefixed the URL to the filesystem.squashfs with "/cdrom"
and failed to find the file.

This is still an issue for me on 15.04. I have to alt-f1, log in and

I don't use ecryptfs, I don't have several copies of gnome-screensaver

In my situation I've got for example the following;
eth0 (root device)
eth0.100 (vlan 100)
eth0.1024 (vlan 1024)
To even get ISC to start, I have to assign a scope to the eth0 device. It
refuses to use either VLAN interface as a valid interface. If I place a DHCP
pool and an IP of 192.168.
This occurred for me with an LTS upgrade from 12.04 to 14.04 on a server
with a static IP.

I found other issues with the current version after updating. For me
