Namaskar again! Here, the main thing concerns about your Railway concession certificate as your subject line indicates. Applying using UDID card is just part of the process. Whether UDID card has doctor's name is another different thing altogether. It could be discussed in separate thread if desired. Hence, we cannot club these things together. Even railway people here acknowledge that doctor name and registration number is not mentioned on UDID card. They admit if one uploads UDID, name and registration number of the doctor should not be asked. So, finding that name using diagnostic sheet etc is going away from the main subject of the matter that is Railway concession certificate. Our focus should be accepting UDID card by railways as it is. I suggested them if one (blind person) applies using UDID, these columns of punching doctor's name and his/her registration number should disappear from the application or it should allow one to type some random numbers or NA etc as I did. Just see, the simple thing in the subject of Railway concession certificate is taking us towards UDID, what is printed on it, doctor diagnostic sheet, about Aadhar kyc UDID etc etc. The focus of the discussion goes wayward, which shouldn't be the case.
