Yes it is right atleast UDID medical certificate should have Doctor Name & Registration Number, even while filing 10A form in ITR Return there doctor name is to be mentioned, there itself I tried and searched in Swavalamban portal and searched Doctor Diagnostic Certificate. On the other hand Railways are unaware of this UDID medical certificate they are just in old era that while we were using concession certificate there we had to mention Doctor Name & registration number the same they are continuing in Railway card application also. We all should tweet or write to Railway Ministry regarding this issue.
