I've finally managed to fix this by simply installing the newest version
of bcmwl-kernel-source (6.20.155.1+bdcom-0ubuntu6) package.
Here's the solution step by step:
1. Download the package from:

2. Reboot to non-working version of kernel

I also had this problem in openSUSE 11.4 64 bit with KDE 4.6.0.
I fixed it for me by:
1. removing files: ~/.gtkrc-2.0-kde4, ~/.gtkrc-2.0-kde
2. Go to KDE settings > "Application Apearance" > "GTK styles and fonts"
3. Select QtCurve theme
