This was pilot error - I mistakenly read the screen which prompted for
username as asking for the iSCSI initiator ID, and had entered a value
on that screen, which explains why the installer insisted on a password.
I have repeated the install leaving the username field empty, and was
not prompted f
This issue appears to be similar to launchpad bug 524637, so maybe the
package should have been 'grub-installer' instead of 'base-installer'

console by adding 'console=tty console=ttyS1,115200n8' to the kernel
command line. The installation proceeded correctly, however on first
