I hate to resurrect this thread but exactly the same error is appearing in a
fresh install of Xenial 16.04.1 with a fresh install of SquirrelMail.
The login page says it 1.4.23SVN, but the i18n/php and strings.php (the ones
that seems to cause the problem don't look the same as the current SM SV
Also the same thing happens on the 32bit Server version. ISO's MD5 is
correct. ISO appears to install correctly to USB flash drive, but
booting with the flash drive and running the inbuilt integrity checker
shows an error in file -
./install/netbook/ubuntu-installer/i386/pxelinux.cfg/default

This isn't a Flash issue, this is a sound issue. If I start up Totem Movie
player and play a video, then start Amarok, Amarok gives an "xine was unable to
initialize any audio drivers" error...
So it's not Flash, it's having two programs trying to make sound at the same
time...

I have this problem too.
In System/Prefs/Sound menu, changing the Sound Events, Music and Movies,
and Audio Conferencing to the PulseAudio Sound Driver, fixes YouTube
videos in Firefox, but breaks Amarok, which gives a "Xine was unable to

I think this have something to do with NFS. The high bitrate mp3 was on
an NFS share on a networked server, when trying to write the low bitrate
output back to the NFS share (all permissions where OK) SoundCon

This is also happening on my desktop (Nvidia graphics) and laptop (Intel 915
graphics).
It's when the Compiz Desktop Effects are turned and is specifically related to
the Wobbly Windows feature.
Install compiz-settings-manager and turn off wobbly and the defect
disappears.
