I think you have been hit by the bug that was discussed on the OpenLDAP mailing lists last November under the subject "crash: cyrus-imapd -> sasl -> pam -> pam_ldap -> libldap-2.x -> sasl". The main problem is that both imapd and libldap try to use the SASL library, but the Cyrus SASL library is not reentrant. I think that currently there are no other solution than to stop using pam_ldap directly and use the pwcheck_ldap daemon instead.
