Hi Justin It's a good idea to have a regular "clear" state update on podlings.
The mission of the incubator is to "guide" on the Apache Way. So the phrasing is important to communicate with the podlings: the podlings should not see this checkpoint as a "sanction" (regarding the score) but more as an update how we can provide better help and service. So, instead of score, I would suggest "help needed". Imho, the template should include also information about the services provided (by the incubator and the foundation) and the podling uses or not.
