I have not seen an ITSM upgrade take 48 hours (yet), but in my experience I have seen them take up to 23 hours to complete. I just upgraded last week and over the weekend on a new staging server, and the upgrade from 7.0.03.009 to 7.6.00.001 took 17 hours, and from 7.6.00.001 to 7.6.03 took 9.5 hours. A test run last month directly from 7.0.03.009 to 7.6.04 took 14.25 hours, on an 8-core 12 gb RAM server with a local SQL Server.
Basically, if your hardware isn't powerful enough, or your database performance is a problem, it will take longer. Our current tests are with two 4-core 8 gb RAM servers, one for AR, one for SQL Server. It is a very resource-intensive process, so make sure that you are giving it enough room to work.
