moved from archive.ubuntu.com to old-releases.ubuntu.com, and update-
manager telling me at every turn to upgrade to a newer release without
giving me the opportunity to do so.

I believe we now have reduced the number of kernel flavors so that this
bug doesn't occur anymore. intrepid has -generic, -server, and -virtual
kernel flavors, jaunty seems to add -rt. Upgrades from release to
release would seem to me to retain the user's preferred flavor. Thus,

Nobody else seems to have had this problem. The stack trace did not make
any sense to me. I suspect it may have been a random error, caused by
something else in the system misbehaving, possibly the hardware.

Intrepid host, Debian etch and sid guests, and possibly other guests.
I've now converted all my guests to use virtio, and things seem to work
better, or at least "apt-get upgrade" no longer stalls if there's more
than one or two packages. I am currently running the kvm from your PPA
that I download
Bernhard, yes, that's pretty much exactly what happens: if a package is
no longer available in the package sources apt has, it's considered
cruft. That's the best heuristic we have.
The whitelist is already implemented, and has been for a while. It is
maintained manually by the user.
