I find this happens most of the time when the system has been suspended
for a longer period of time. For shorter periods of time, it seems that
the system is able to resume correctly upon opening the lid of the
T400s.
I know that it suspends correctly because the "sleep"/susp
appears to be repeatable - happened again after rebooting and logging
in. Occurs when launching a program from the tray.

Was running 0605. I'm still getting the sound problems with bios version
0902. I get sound down to about 56% (-15.27.dB) - just barely audible,
meaning that with the combination of the pulseaudio updates and this
bios update, the sound range has improved, but still isn't really where
it ought to be
