Hmm, running the current setup (200ms sleep+iwlwifi unload-on-suspend), I must say that the bt loading has become more reliable. With kernel 4.19rc7 and the kernel parameter as you prescribed, the BT adapter won't work at all. The timeouts that would only occur sporadically after resume, are now present from boot, and the previous unload-modules/suspend/resume/load-modules cycle no longer works to get BT back on. Adding to that, there is a (somewhat) reliable way to get bluetooth back. It involves goign to the following process _twice_: stopping the bluetooth service, unloading all relevant modules, suspending the laptop, resuming, reloading all modules, starting the bluetooth service. Rather cumbersome, but it might help other experiencing this issue. This seems similar indeed, although I don't have any thunderbolt hardware. Public bug reported: In recent versions of initramfs-tools-core, the hibernation resume script (/usr/share/initramfs-tools/scripts/local-premount/resume) is no longer +x, so it's silently skipped during boot, causing resume after hibernation to fail. I just encountered this bug again in cosmic with kernel 4.17.0-041700rc1-generic. Definitely not fixed in 4.17. Note that with kernel 4.15, the xhci hang occurs too, but then I can reset the bus without causing a kernel Oops.
