It is worth it.

It has been a while since I did the analysis at one ISP I
admin for, but based on a post-analysis of 10,000 rejects on
about 70,000 total offered pieces across a 3 or 4 day period,
I had 3 complaints.  Server load was and remains dramatically 
reduced.  But then, I had complete support from the owners of 
the ISP.  

At another site, in 3500 users, only 2 domains, and maybe ten
users decided to 'opt-out' and be a 'spam-friend' (see the
README.cf for sendmail for the term's meaning ...)
