What's the problem? You can either install from CPAN, source, or rpm. Unless you're into modifying the scores or adding new rulesets that's it for the installation. You may also wish to install razor2 and/or pyzor. Spamassassin will automatically use these tools to enhance it's operation. You then need to use a procmail recipe (in the doc's at www.spamassassin.org) to pass mail through spammassassin and filter for messages marked as spam. Bottomline is that there isn't a lot to making it work. Sendmail isn't modified at all.
