Fixes #639. The JSON handle validator now determines the result first and invokes the async-validator callback exactly once. JSON mode accepts objects and rejects empty, malformed, null, array, and primitive inputs; the structured form keeps its existing behavior. The regression tests explicitly assert a single callback for valid, invalid, and non-JSON-form inputs.
