This time I got it right. The USB vendor:product description needs to be in the blacklist section of the btusb file; it only worked by chance the first time I tried; now it works after a reboot. (We have to prevent btusb from managing this before its firmware is loaded or whatever, see ath3k on wireless.kernel.org)
