Update: I was wrong. While testing the issue I only restarted my machine which did not power cycle the Bluetooth radio, so it probably retained its firmware between reboots and it worked regardless of the kernel I used. It turns out a proper shutdown and restart re-introduces the bug on 3.16.* (unpatched), but Bluetooth is indeed working on 3.19-*. Also, the version of linux-firmware has nothing to do with it. I'll now tag this bug report accordingly; that is, add the 'confirmed' and 'kernel- fixed-upstream' tags.
