Review Comment:

   I merged the two constructors into one, but didn't add a split 
`PerSegmentCorePKLookup` for cache. Since I just cached the index(Integer) of 
enums array in current version, the cache is: `Map<IndexReader.CacheKey, 
Integer> enumIndexes`.
