I've since discovered that to trigger the bug you have to start virtualbox-ose-qt, and start a virtual machine (it doesn't seem to matter what's installed inside the VM -- even a blank one will do it). The VM window will show the bug but nothing else will. Then if you quit the VM and the virtualbox GUI, any qt app started after that will show the bug, until you restart the X server.
