Just catching up on this, I should add that the steps followed in my Comment #12 are using KDE4 versions of Dolphin and Kate, no KDE3 programs in use to reproduce this bug. I'm not sure if the fix mentioned would then apply to these steps ? I'm also not sure where "edit your icon settings" is, exactly.
