Originally saw this issue on 6.14.0-28-generic so I went back to booting 6.14.0-27-generic. I even tried 6.16.4 from mainline but it happened there too. 6.14.0-27-generic seems the most reliable. Diagnosed this as an amdgpu bug with the help of Google Gemini.
