The failure is this:

 Running command ['lvremove', '--force', '--force', 'ubuntu-vg/ubuntu-lv'] with 
allowed return codes [0] (capture=False)
   WARNING: Not using device /dev/sdby2 for PV 
udw8jB-AL4V-3M57-8ot6-7uz3-HCgt-W2Srx0.
   WARNING: PV udw8jB-AL4V-3M57-8ot6-7uz3-HCgt-W2Srx0 prefers device /dev/sdbe2 
because device is used by LV.
   Cannot update volume group ubuntu-vg with duplicate PV devices.

which I've never seen before, but we've made some changes to avoid
running lvm-scanning commands at the same time as lvm-changing commands,
which has been observed to cause problems, so maybe the next release
will fix this...
