I've seen that before. It looks like multipath is available, and
possibly old installation (partial or not) ubuntu-vg/ubuntu-lv are
available. They need to be carefully cleaned up, with VG UUID, LV UUID,
PV UUID specified, to ensure that lvm tools clean things on the
assembled / used paths, and doesn't randomly try to search for them.
Because multiple paths will be found, and lvm tooling gets confused.
This is similar to dissassembled raid1 with metadata at the end of the
disk, where to lvm, it looks like there are a multiple identical PVs
which must never be true.
