I don't think an append option is all that useful to be honest.  If you have
two different dns servers (eg one for the real world and one for an internal
network) and ask whichever is first server it'll either give you an answer
or tell you the domain doesn't exist at which point it'll give up.  You'd
need to run a proper dns server to have domain-specific lookup rules.

What I think is needed is to have an option to either modify DNS to follow
the VPN or not.  Ideally, if the VPN doesn't give any DNS servers, the old
ones should be automatically preserved.
