The error shows that it's trying to boot grub-efi, and can't find the
modules for it.  If the installer was booted in CSM mode, this /should/
cause the installer to install grub-pc, not grub-efi; so I wonder if one
of your other install attempts installed grub-efi, creating an EFI
system partition, and the post-install reboot is loading this grub-efi
from the EFI partition instead of grub-pc from the MBR.  You could try
deleting EFI/ubuntu/grubx64.efi from the machine's disk, and see if that
makes a difference?
