I'm not sure if this is a further advancement of this problem, but I've noticed that ctrl-w and ctrl-F4 don't work when a plugin has focus in Windows. This means you are trapped unless you can find a place to click, or you close FF entirely. At the very least, there need to be some keys/shortcuts that are off limits to all plugins. Shortcuts to close the tab and window should be at the top of that list.
