Experiencing the same thing here. Hardware is a Lenovo Thinkpad X301.
The only issue I ever had attaching USB devices to the machine was that
the first device attached would fail, but every subsequent device would
attach with no issue.

As of today, however, both with Karmic and - post-upgrade - Lucid, I am
now experiencing the same condition:

This [https://answers.edge.launchpad.net/ubuntu/+question/4272] claims
that removing the USB 2.0 module [modprobe -r ehci_hcd] will correct the
condition, but this is clearly a suboptimal solution.

I can't be sure what caused the condition after a year plus of usage,
but I can confirm that after multiple reboots and even a distribution
upgrade, the problem persists.
