This is actually two-fold... first the notification-frontend (notification-plugin in unity8) needs to change the used UI-elements for additional actions (> 2 actions).
Secondly, the telephony-service needs to change the actions passed to a snap-decision notification used for the "incoming call"-use-case. This combo button has two hit areas which caused a lot of confusion with users. One part DECLINES the call the other expands for additional options. Please remove combo button and replace with a toggle button with only one hit area. Hitting that button will reveal an expansion with additional options to users for a quick reply. Remove the CUSTOM option in the quick reply menu.
