I haven't been able to identify an action or series of actions which will recreate this reliably. Notably, I recently updated to Ubuntu 11.04 from 10.10, and Compiz/Unity appears to have been having other, seemingly unrelated issues handling mouse input (losing bindings for screen corners, namely).
