Some more info. I verified that my printer is working, rebooted ubuntu,
tried again (all pages, even only). Yelp did send a print job to CUPS
-- it printed pages 3, 7, and 11 before I canceled the job. (I was
expecting pages 2, 4, and 6). I'm now printing all 145 pages without
getting fancy and

10.04 iso on a running 10.04 system. My workaround was to run the
program as superuser (gksu usb-creator-gtk). The iso was not moved from
