I have been running 2.6.38-11 for 6 weeks now, and there have been no more
occurrences of the problem.

The cx88 driver patches have fixed the problem for me.

I have no way to reproduce this. It always just occured randomly every
3-6 days. But I don't think I has happened for the last two weeks or so,
even with 2.6.38-10-server (now upgrading to 2.6.38-11).

I have just installed 2.6.38-11.48 and the problems seem to be gone.
I will post another comment after some more extensive use of the new kernel.

The kernel in -proposed (2.6.38-11.47) has several patches for the cx88
driver; see https://wiki.ubuntu.com/Testing/EnableProposed in case you
want to give it a try.

Looking into the changes from 2.6.25 to 2.6.38 there were many locking
changes, which could be the problem:

After reviewing the dmesg files I posted, I can see a common call path to the
problem:
v4l2_open -> video_open -> mutex_lock
The path is always related to the cx88xx drivers, for a DVB card.
Is it the DVB card drivers?

This same problem just happened to me while playing back a video file
with vlc. After a few seconds into playback sound suddenly stopped and
video got really choppy until it stopped completely and a lot of disk

I get only this in dmesg:

The same problem here. Which system files may be relevant?
It mostly happens when vim tries to overwrite a file it edited.
