I have a similar problem, with my NVIDIA MCP78 / ALC662 soundcard

Alsa detects the devices and can play sound, but pulseaudio sometimes
doesn't see the devices.

Ok, well, I am glad you found a solution. Maybe you could submit your
profile configuration to the pulseaudio group. That way, Ubuntu can
make sure that your kind of hardware is properly configured in the next
Ubuntu release.

After poking around ubuntuforums.org I found something that got my
Audiophile 2496 operational, it took creating a profile for it and
loading it into pulseaudio:

I remember having this problem with earlier versions (7.04 etc) but
I don't know if this will fix your problem, but try installing the
'linux-backports-modules-alsa-karmic-generic' package and reboot.
