I have a similar problem, with my NVIDIA MCP78 / ALC662 soundcard (complete config: http://www.alsa- project.org/db/?f=539b8b2005922af2774faa3c85d8c208dea79425)
Alsa detects the devices and can play sound, but pulseaudio sometimes doesn't see the devices.
