I have a similar problem, with my NVIDIA MCP78 / ALC662 soundcard
(complete config: http://www.alsa-
project.org/db/?f=539b8b2005922af2774faa3c85d8c208dea79425)

Alsa detects the devices and can play sound, but pulseaudio sometimes
doesn't see the devices.

My temporary workaround was to disable hardware autodetection in
/etc/pulse/default.pa

   enabled static alsa module:
     load-module module-alsa-sink
     load-module module-alsa-source
   disabled autodetection:
     #.ifexists module-udev-detect.so
     #load-module module-udev-detect
     #.else
     #load-module module-detect
     #.endif
