I had this problem, and it was fixed by doing this [1], that is, using alsamixer, muting the second volume "S/PDIF" (highlight it and press M). That is, if the digital out is not muted, analog is muted (??). Had fallen back to crappy integrated audio for months on 10.04, until it happened once agai
