xserver-xorg-video-ati 1:6.10.99.0-1ubuntu1 seems to have fixed this
bug. I can't reproduce this issue anymore, switching it EXA seems to
have fixed it. Thanks!

Hi Nick,
First thing to check - have you ever had the -fglrx driver installed?
If so, leftover bits can sometimes cause problems like these. See
https://wiki.ubuntu.com/X/Troubleshooting/FglrxInteferesWithRadeonDriver
for directions on removing it completely.
