Same issue here on a Thinkoad P70, as a workaround I keep switching airplane
mode on & off until the Wifi adapter gets detected again. Using bluetooth on
this laptop is a bit erratic too

I had a similar issue after restarting a Lenovo thinkpad T520 (iwlwifi on a
Centrino 6205).
After a cold start, not all the time. Sometimes I needed up to 4 restart before
the wifi connexio
I'm also seeing the same problems running Ubuntu 16.04 on an Intel
NUC6i5SYK. I installed Ubuntu a couple of days ago starting with kernel
4.4.0-21-generic, here the wifi worked most of the time. Updating to
kernel 4.4.0-22-generic made this problem even worse. Now wifi doesn't

I was able to trigger this again today on 4.4.0-16:
[ 13.665350] iwlwifi :04:00.0: Unsupported splx structure
[ 13.678584] iwlwifi :04:00.0: Direct firmware load for
iwlwifi-8000C-19.ucode failed with error -2
[ 13.679439] iwlwifi :04:00.0: Direct firmware load for
iwlwifi-8000
I tested 4.5.0-040500-generic, did 20 reboots/shutdown cycles, both
powered and on battery. I haven't been able to trigger the issue again.

I am on Debian Testing and have the same problem with kernels 4.3 and
4.4 (Debian one's) as well as 4.5 (compiled from unmodified source with
Debian config). Sometimes I have to reboot around 10 times to get a
connected with NetworkManager, although the device seems to be available
through iwconfig
Did this issue start happening after an update/upgrade? Was there a
prior kernel version where you were not having this particular problem?
Would it be possible for you to test the latest upstream kernel? Refer
to https://wiki.ubuntu.com/KernelMainlineBuilds . Please test the latest
