I haven't seen this problem for a while, but it was a race, so I am not
sure if I will be seeing it, or if anyone else will. I will keep an eye
on it just in case.

I made 11 calls in a row to my mako running image #70.
I flashed image #78 this morning, and made another 11 calls in a row to
the phone with no issues.
Were you doing other testing on the phone before this problem
manifested, or did you run this test after a fresh boot? Just trying
to g

Could you try the latest stable system image ( #70 ) and see if this
occurs? I haven't seen this behavior at all on a Nexus4.
I will try and reproduce tomorrow as well.

Looking at the syslog file, a few interesting things:
Network Manager / oFono failed to activate your data connection,
following by ofonod terminating itself:

Adding syslog from the device. From 15:30 onwards is when this behaviour
was observed.
