Looking at the syslog file, a few interesting things:

Network Manager / oFono failed to activate your data connection,
following by ofonod terminating itself:

When ofonod got restarted, network-manager crashed with a segv, making
it to respaw itself as well:

Sep 27 15:41:08 ubuntu-phablet ofonod[8536]: [UNSOL]< UNSOL_RIL_CONNECTED

Sep 27 15:41:08 ubuntu-phablet NetworkManager[8558]: <info> NetworkManager

Following that NM/ofono fails again to start the data connection, but
nothing bad happens.

I believe that during this entire crash scenario, telepathy-ofono lost
track of ofono somehow. But it'd also be good to have more logs from
ofono to know if the calls were actually happening in that level.

Please add more debug to oFono by default when testing, and also see if
you got something at /var/crash, so we can see if we can get more
information about this bug.
