Looking at the syslog file, a few interesting things: Network Manager / oFono failed to activate your data connection, following by ofonod terminating itself:

Following that NM/ofono fails again to start the data connection, but nothing bad happens. I believe that during this entire crash scenario, telepathy-ofono lost track of ofono somehow. But it'd also be good to have more logs from ofono to know if the calls were actually happening in that level.
