I can't see anything the driver is doing wrong. It looks like the
problem might start midway through the scan, where you seem to stop
receiving frames even on the operating channel for your AP. As far as I
can see the driver is setting up the hardware the exact same way but
getting different result

I've looked over the driver and can't find anything obviously wrong. As
far as I can see the device ought to be getting configured the exact
same way when it fails as when it works.

It appears that once in a while after doing a background scan you're no
longer seeing beacons from the AP. When that happens mac80211 actively
probes the AP but is not getting a response that way either. Then it
gives up and sets the wireless state to disconnected. Reconnection
attempts are similar

It looks like the trouble starts after doing a scan for APs. The device
seems to get set into an idle mode that's normally used when not
associated to an AP, but then it's also trying to communicate with the
AP. I can't see why this is happening though.

I tried the dongle in one of the usb hubs in the back of the computer,
and experienced the same issue, so I don't believe that the problem is
related to the specific USB hub or port.

Bug #225851 appears to be reporting problems with the same hardware
going as far back as hardy, so in all likelihood this isn't a recent
regression.
Rick: When you get a chance please do the following. Install the trace-
cmd package on the affected machine and run 'sudo trace-cmd record -e
mac8021
I don't recall this happening before, but I haven't used this computer
for about 2 years, sorry.

Like mentioned in comment #3, it would be good to know if this just
started happening in Quantal or a recent update. If it did just start
happening in Quantal, I can perform a kernel bisect to identify the
offending commit.
