I solved. I re-installed Ubuntu 12.04.2 and I blocked the packages:
- linux-generic-lts-quantal
- linux-headers-generic-lts-quantal
- linux-image-generic-lts-quantal
from 3.5.0.23.30 to 3.5.0.26.33
I think that this packages have the bug.
If I install this packages the bug appears, and if I remove
I formatted the PC, I installed 12.04.2: no problem. I upgraded just
with main repo and precise-security repo and the problem occurred again.

The problem is more generic. Sometimes you click the close button (upper left
'x') and, if any, the underlying window gets closed insted of the foreground
one. It is unpredictable wheter it is going to happen or not, but it can be
very dangerous (for example google chrome doesn't ask for confirm
