fix as per below:
For the record, after mounting rw and entering safe mode, I used the
dpkg fix broken pages menu item which noted that there was a lot of

I have a similar problem after upgrading from Jaunty (the upgrade crashed and I
was able to finish it using a bootable USB). mountall --debug appears to hang
because other mounts are waiting for / e.g.:
try_mount: /tmp waiting for /
/ mounts perfectly when I use http://upstart.ubuntu.com/
