Ah, so I think you may have the winner. I set my temp directory to be
something other than /tmp, and turned ClamAV back on, and it's been running
for about an hour now with no obvious ill effects. I will report back if
something else crops up, but I think this may solve it.

Things haven't really changed for gpdftext.
It's not impossible for someone to adopt the package in Debian and the
upstream at the same time, it's not a lot of code. However, it seems
that this little niche was a bit too small for anyone else to want to
pick it up once I stopped needing the code.
