I've been having a similar issue to the one describing which I've traced
to the use of 'module-suspend-on-idle'.
My Debian release and package version are quite different but as it
might be the same issue I'm adding some information to this bug before
submitting a new one.

For the record, I have also observed this behavior in jessie. Pulseaudio
will mute when coming out of suspend, but only occasionally. I'll try to
get some logs.
