I had the same problems with my Xircom Cardbus Ethernet 10/100 NIC and
found out that adding
allow-hotplug eth0
to the /etc/network/interfaces works.
Strange to say, that my 3com 3CRWE62092B WLAN NIC even works without
addi

I also saw this problem. Somehow the Debian-specific section of
network.opts got lost in one of the two recent NMUs, since -5 still has
that part of the script. Trivial patch to revert to the script of -5 is
attached.

I don't have hotplug installed, and cardmgr is not bringing eth0 up anymore. I
did:
1. Unset REFRAIN_FROM_IFUP in /etc/default/pcmcia
2. Added 'allow-hotplug eth0' to /etc/netork/interfaces
But cardmgr doesn't bring the interface u
