The temporary workaround for this to work is to add:
always_direct allow all
to squid.conf, but I'm afraid that with this, the cache feature
doesn't work.

I'm having the same problem, this dump below can be seen exactly the
same on both ends of the pppoe session, this implies that the server's
LCP responses never make to the wire:
