Before RH made this easy with their authconfig utility, I wrote a short
doc that explains how to set this up manually.

Besides giving the steps and some examples, it describes some of the
issues. You're probably running into an issue about "required" and
"sufficient" checks in the "auth" section of /etc/pam.d/ssh.
