I can think of a few reasons, and also the guys that code Sendmail have a
few -- they did not impliment this configuration method out of boredom.

If you do not feel like getting to know each .cf file with each release,
then use the .mc and m4, I mention this because the original poster was
probably indicating they do not know Sendmail well, and if they did, I
cannot think why they would be editing the .cf directly.

Needless to say, if you know the .cf file inside out, and prefer to modify
it directly, then why not indeed, you have that choice.
