Well, I can't tell you what the problem is, but I will offer a solution that
has worked for me a few times in the past with various problems of Outlook
and mail.
Have your client download all his mail (check it or whatever).
Delete his mail spool file.
"touch" a new one
"chown" it back to the original permissions
See if it helps.
This has come from trying to (unsuccessfully) diagnose Outlook problems with
getting mail. Various types of problems have occurred and all of them so far
have been fixed by the above procedure. Most of them have never recurred.
I have decided that there are some sorts of anomalies that occur with
the immense flow of mail across the Net, and figure that Outlook can't
handle them. Funny, but no other mail program as ever had a problem ;)
