perhaps you cd's into the cdrom hierarchy, and then su'ed to some other user ?
that would hold the cdrom open until that process terminated.

similarily, spawning off a process from the cdrom directory hierarchy
can also leave the cdrom "open", and un-umountable, until that process
terminates.

it's more than likely one of the above.

there are also some utilities to help you in locating processes on
filesystems (lsof - list open files, and another, but I forget which)
which can be helpful in determining what files are open on which file systems.
