Well, Selena Sol's and Matt's script archive both have sets of Perl CGIscripts that
do this kind of thing so they might be good places to start.
I don't have the URLs to hand right now, but your favourite search
engine will find them.
It sounds like you can do what you need with the HTML Forms tag, and
a Perl CGI script, anyway.
